Position Overview
We are seeking an experienced Registered Nurse (RN) to join our hospice team as a Weekend Phone Triage Nurse. This vital role provides after-hours clinical support, guidance, and triage for hospice patients and families via phone. The nurse will assess patient needs, provide education and reassurance, and coordinate care as needed. While this is primarily a remote, phone-based position, occasional in-person visits may be required for urgent situations.
This role is ideal for a compassionate, autonomous nurse who excels in assessment, communication, and crisis management - and who values the flexibility of weekend, per diem work.
Key Responsibilities
Phone Triage & Clinical Support
Serve as the primary point of contact for hospice patients and families during weekend hours.
Conduct thorough symptom assessments over the phone and provide clinical guidance, education, and emotional support.
Determine when an in-person nursing visit or escalation is necessary.
Coordinate care with the interdisciplinary hospice team, including physicians, aides, and case managers.
Document all triage calls, interventions, and follow-up actions in the EMR promptly and accurately.
In-Person Visits (as needed)
Respond to urgent patient needs that require in-person evaluation or intervention.
Administer medications, manage symptoms, and provide comfort measures consistent with hospice protocols.
Communicate changes in patient condition to the appropriate on-call or case management staff.
Qualifications
Graduate of an accredited School of Nursing
Active RN license in the state of Minnesota
Minimum 1 year of hospice or home health experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication and interpersonal abilities
Compassionate, calm, and patient-centered approach
Reliable phone and internet connection for remote triage duties
Valid driver's license and insured vehicle for occasional visits
CPR certification required
