As the Nurse Navigator , the navigator in collaboration with other PHH Services, provides and coordinates all aspects of treatment, evaluation, support services and education for patients receiving service within the Integrated Recovery department. Provides clinical and admistrative functions. Participates in maintaining a safe, cost-effective patient care environment.


QUALIFICATIONS:

• HIGH SCHOOL DIPLOMA OR EQUIVALENT REQUIRED
• CURRENT LPN LICENSURE IN THE STATE OF PENNSYLVANIA REQUIRED
• UNDERSTANDING OF PAYOR QUALITY GUIDELINES, MIPS INITIATIVES AND PAY FOR PERFORMANCE PROGRAMS PREFERRED
• AT LEAST TWO YEARS EXPERIENCE IN PHYSICIAN PRACTICE SETTING REQUIRED
• LICENSED IN THE COMMONWEALTH OF PENNSYLVANIA, CPR, EXCELLENT VERBAL COMMUNICATION SKILLS
• KNOWLEDGE IN REGULATORY REQUIREMENTS OF THIRD PARTY PAYORS SUCH SUCH AS MEDICARE, MEDICAID AND BLUE SHIELD
