Job Description: Are you a skilled and compassionate

Key Responsibilities:
- Provide skilled nursing care in accordance with care plans and physician orders
- Assist with medication administration and wound care
- Monitor patient conditions and report any changes to the RN/Clinical Director
- Collaborate with interdisciplinary team members to ensure quality outcomes
- Educate patients and caregivers on treatments and wellness practices
- Document care and observations accurately in the EMR system
- Communicate with physicians, case managers, and other team members as needed
Qualifications:
- Current LPN license in the state of Indiana (required)
- Minimum 1 year of home health, hospice, or clinical nursing experience preferred
- Strong assessment, documentation, and time-management skills
- Reliable transportation and valid drivers license (for field visits)
- Passion for helping others and making a difference in patients lives
- EMR experience a plus (Axxess preferred but not required)
