Description of the role:
This role requires a Licensed Practical Nurse (LPN) to provide direct patient care and support in a home care setting in Wayne County, MI.
Responsibilities:
- Conduct patient assessments and develop care plans
- Administer medications and treatments, and medication setup
- Monitor patient progress and communicate with healthcare team
- Provide patient education and emotional support
Requirements:
- Valid LPN license in the state of Michigan
- Some one that is able to travel around Wayne County
- Strong communication and interpersonal skills
- Ability to work independently and as part of a team
