Community Outreach & Admissions Coordinator  who will be the main point of contact for families and individuals seeking services from Mosaic. This isn't just a job; it's about being a crucial part of a team that provides people with a meaningful life in a caring community.

What You'll Do

✅  Serve as a Connector  – Serve as the main point of contact for all referrals, screenings, and admissions. You'll coordinate teams to conduct screening meetings and visits with families seeking services.

✅  Drive Admissions  – Oversee the entire admission process, ensuring all requirements are met before and after an individual is admitted. You'll also develop systems to monitor progress with growth and admissions.

✅  Engage the Community  – Educate the community about Mosaic's services through outreach with case management, schools, and parent groups. You'll also advocate for Mosaic by serving on city or county resource groups as needed.

✅  Develop a Model  – Create an agency-wide model for intake, referral, and admissions to ensure consistent and thorough processes.

Who You Are

✔  An Experienced Professional  with at least two years of experience working with individuals with developmental disabilities.

✔  A Strong Communicator  with excellent written, oral, and public speaking skills.

✔  A Problem Solver  with strong planning and organizational skills who can prioritize work and meet deadlines.

✔  An Advocate  who is committed to a philosophy of services that empowers people.

 

SALARY : $57,500 - $60,000

POSITION CLOSES: October 2, 2025 

What You'll Bring

  • Bachelor's Degree in Psychology, Special Education, or a related field.
  • Minimum of two years of experience working with individuals with developmental disabilities.
  • Knowledge of habilitative services for individuals with developmental disabilities.
  • The ability to travel within the state and work in an on-call capacity if needed.
  • Availability to work a flexible schedule that may require evenings or weekends to meet the needs of the community and families.
