Serve as an on-site HealthySteps specialist, providing care coordination and connecting families to critical resources.
Complete training in HealthySteps and maintain the program's standards. Training will be provided.
During well-child and office visits, provide families with universal screening using tools like PSC-17, SWYC, and ASQ-SEs, and assist with completion of the tools when needed.
Connect children and families with the right level of support based on their screening results.
Utilize the Social Determinants of Health to refer families to resources and tele-behavioral health services when necessary.
Help carry out the Help Me Grow program and serve as the central contact person.
Collect, manage, and interpret data and provide reports to management.
Allied Behavioral Health Solutions is a locally owned organization focusing on collaborating with individuals, families, medical providers, and communities to create better outcomes for all. This role is a collaboration with Allied Behavioral Health Solutions and a local, high volume pediatric clinic where the Community Health Worker will be employed by Allied Behavioral Health Solutions but will spend their day working in the clinic with providers and children and their families to complete screening tools, connecting families with support, and tracking data.
Minimum Requirements:High School diploma required (Bachelor's degree preferred)
Proficiency in Microsoft Office (Word, Outlook, Excel) and Google Drive
Experience in a medical and/or clinical setting preferred
Excellent oral and written communication skills, demonstrating professionalism in all forms of communication.
Strong organizational skills and self-motivation.
Ability to work effectively within an interdisciplinary team.
Capacity to collect, analyze, and report data, and engage in feedback loops.
