Serves as a licensed behavioral health clinician who works with the primary care clinical team that advances behavioral health integration in the primary multigenerational family care setting using the principles of the Collaborative Care Model. The clinician is part of a patient-centered care team that provides measurement-based treatments with definitive outcomes. Care provided is population and evidence-based care, and the clinician is responsible for the quality of care and the outcomes of their clinical work.
The clinician uses their skills in collaboration with a psychiatrist, along with the physical health specialists on the team to address both the physical and behavioral effects of a disease, illness, or injury.

Job Responsibilities
- Provides evidence-based behavioral health interventions and education
- Assesses patients for appropriate therapeutic modality and is able to provide cognitive behavioral and other therapeutic services
- Assesses and systematically collects and tracks data related to patient behavioral health status and works with care team to develop the plan of care
- Utilizes data, including registries, to continuously meet care coordination needs across settings in conjunction with the patient, family and/or other care providers
- Develops and maintains a working knowledge of community resources and managed care review processes
- Assumes responsibility and accountability for effectively managing behavioral health care of individual patients
Qualifications
- Master’s Degree required, or equivalent combination of education and related experience.
- Currently licensed clinical social worker (LCSW), licensed professional counselor (LPC), marriage and family therapist, or psychologist eligible to practice as a Mental Health Counselor in Wisconsin, plus specialized certifications as needed
- Minimum 3 years of relevant experience, required
- Excellent written and oral communication skills with the ability to adapt communication style to address the needs of individuals at all levels
- EMR experience and working knowledge of Microsoft Office applications required
- Core philosophy or values consistent with a patient-centered approach to care
